Individual Observer Application(Closed)

The Fukushima Ministerial Conference on Nuclear Safety opens applications for individual observers.
1. Those who wish to attend the Conference are required to consent to the precautions noted below, fill out the application form and submit it, during the period from 12:00 (noon) on Thu, November 29, 2012, through 12:00 (noon) on Wed, December 5, 2012, (Japan Standard Time). Applications will not be accepted unless you have consented to the precautions noted below.
Precautions
- You are prohibited from carrying any placard or banner or wearing any brow-band, sash, cloth bib with number, name or logo or clothes showing political message in the venue. You are also prohibited from putting up any poster or distributing any literature there. In the event that any of the above-mentioned items are found by security checks carried out before you enter the venue, you will be required to leave them at the inspection station. In some cases, admission may be denied.
- While in the venue, you must refrain from talking and sit quietly during the Conference at the assigned seat. You must keep quiet without interrupting the proceedings and management of the Conference. You are prohibited from making any remarks at the Conference. You must refrain from talking loudly in the venue, which would cause discomfort to other participants.
- You must follow the organizing staff’s instructions while inside the venue, including any instruction to leave the venue in the event of a violation of any of the above.
2. Up to 50 persons are permitted to attend the Conference as an individual observer. Attendance permits will be issued on a first-come-first-served basis to up to two persons per organization/entity.
3. Notification of issuance of an attendance permit will be sent by Mon, December 10 to the email address specified in your application form. The person who receives this notice is expected to have an attendance permit ID issued at the venue upon confirmation of identification. (For this purpose, (1) a copy of the email notice of attendance approval and (2) a passport, driver’s license valid in Japan, or other photo ID issued by authority of the Government of Japan must be presented.) The holder of the issued ID will be permitted to enter the venue after passing through the security check.
4. To observers from abroad: The notice of approval to attend the Conference does not mean that you are exempt from obtaining a visa to enter Japan. You need to apply for a visa in accordance with the ordinary procedures to the nearest Japanese embassy or consulate general of Japan. (The Conference Secretariat will not endorse an applicant. Applicants who need to be endorsed for the issuance of a visa are required to obtain a letter of endorsement from an endorser residing in Japan.)
5. Personal information provided in relation to submission of an application to us will be used only for procedures required for participation in the Conference.
